    This form serves as an agreement that you will receive a calendar at the agreed price of $15.00 per calendar, honoring the promotional rate available during this pre-order period.  Please note: all funds raised will benefit the Central Virginia CrimeStoppers.  Those requesting a calendar to be mailed will need to pay an additional $4.95 in shipping and handling.
