Residents Interested in Covid-19 Vaccinations May Register for Public Safety Clinics Online

RUSTBURG, VA - As the Virginia Department of Health continues to rollout COVID-19 vaccines (for those eligible under Phase 1B), there may be local opportunities for Campbell County residents to secure any unfilled appointments at the Public Safety Department's planned distribution sites for designated 1B categories. 

Interested residents, please complete the survey form located at: http://www.campbellcountyva.gov/FormCenter/Public-and-Employee-Relations-12/COVID-19-Vaccine-Interest-Form-for-Campb-77.  Completion of this form indicates your interest in receiving a vaccination should vaccine supplies and appointments happen to become available in the days ahead. 

Please note that if you cannot be reached, you will remain on the list, however, the next person will be contacted to fill any vacant slot. If contacted, you will be provided with information on the time and location of your vaccination. In addition, those who are contacted will be required to complete and print a consent and screening form, which is likewise attached to the survey form, or may be found here: http://www.campbellcountyva.gov/DocumentCenter/View/7449/Covid-Vaccine-Consent-Form---Fillable  

Again, the submission of this interest survey form does not guarantee a vaccination appointment, but rather places you on a list should one become available. 

As always, any vaccination opportunity coordinated by Campbell County is strictly voluntary and in alignment with the Virginia Department of Health guidelines and protocols.
