[ARCHIVED] Citizens' Input Requested in Statewide Dog Licensing Study

The Virginia Department of Health, along with the Virginia Veterinary Medical Association, animal control officers, local treasurers, and humane groups, is conducting a study of dog licensing in Virginia and would like dog owners' feedback on process.

If you are interested in participating, please fill out this survey and submit it by July 1. The survey form may be printed/scanned and sent as an electronic attachment to Dr. Julia Murphy, Virginia Department of Health at julia.murphy@vdh.virginia.gov or faxed to Dr. Murphy’s attention at 804-864-8131. Surveys may also be mailed to:
